Please note:

• All users must complete the prerequisite training.
• All users must reserve a minimum of one hour.
• Available hours are 8:00am to 4:00pm EST, Monday through Friday.
• The facility must be clean and vacated by 5:00pm daily.
• Cancellations may be submitted with this form.
